Service Description
In-depth 1:1 social services to help facilitate transitional services to include care management, direct referral assistance, housing stability, nutrition, food security, ADLs, transportation, social and community connection, financial wellness, family support, and other non-clinical services such as meals. Staying at a luxury community provides a safe environment while helping relieve the burden of navigating different service systems, leading to confusion, stress, and a higher probability of institutional care and related costs. Continued follow-up after check-out to ensure all resources are being utilized and to help with improved outcomes and hospital readmissions. Our team’s knowledge and experience give our guests and their families peace of mind. With these experts, families with elderly loved ones can know that if they are traveling or live out of state, there is a plan to ensure their loved one is safe and that someone is working diligently to coordinate care.
